The packet includes the information sheet, a packing list, the permission slip and health form (both need to be filled out and signed by a parent), and the authorization for medication form. If your child needs to take any medication at camp, even over the counter, the authorization form must be filled out by a doctor and faxed to the middle school office by Thursday, Sept 9!

Parent Evaluation of Student Led Conferences

To all 6th grade families, this summer we mailed Cispus permission forms to all families of 6th graders.  It’s time to return the forms to the Middle School office any day the first week of school if you haven’t already done so. We need the permission slip, health form and medical authorization form (only if your child needs to take any medication and it needs to be signed by a doctor) in order to be able to take your child to Camp Cispus. If you haven’t paid yet you can still pay in September or make payments all year, but we need the forms filled out and turned in before we leave. 6th grade team

PS – All medications (prescription and over the counter – includes a non-aspirin pain reliever or Benadryl) needed for students while at camp shall have an Authorization form completed by you and your childʼs doctor and the medicine (in its original container) should be checked in at school prior to leaving for the trip, no later than Monday, September 14.  All medications will be locked up and distributed by the camp nurse. EACH MEDICATION  MUST HAVE  ITS OWN FORM.

Sports Physicals for Students: Sports physicals will be available in the HS Health clinic on Monday, August 17 and Wednesday, August 19th. They need to be scheduled from 9:00 – 4:00. Please call the Health Department to schedule this appointment at 385-9400. Students will need to bring a signed consent form and insurance information. If students do not have insurance, there will be a $40 charge and sliding fee scale will be offered.
