Sports Physicals
Sports Physicals for Students: Sports physicals will be available in the HS Health clinic on Monday, August 17 and Wednesday, August 19th. They need to be scheduled from 9:00 – 4:00. Please call the Health Department to schedule this appointment at 385-9400. Students will need to bring a signed consent form and insurance information. If students do not have insurance, there will be a $40 charge and sliding fee scale will be offered.
