Cispus Information
Here is a copy of the 09-10 Cispus Packet in case you lost yours or never got one. Remember that the forms need to be turned in the first week of school, or earlier, because this year we will be going to Camp Cispus the second week of school! We don’t leave until Tuesday, Sept 15, and we need all the forms, permission slip, health form, and the authorization to give your child medication. That authorization form requires your doctor to sign so please get that form completed over the summer before school starts.
