Cispus Information
Click on the link for a copy of the 10-11 Cispus packet in case you lost yours or never got one. Remember that the forms need to be turned in the first week of school, or earlier, because this year we will be going to Camp Cispus the second week of school again! We leave for camp Tuesday, Sept 14, and we need all the forms, permission slip, health form, and the authorization to give your child medication prior to leaving. The medication authorization form requires your doctor to sign so please get that form completed over the summer before school starts.
