This sheet is to help get your student athlete ready for their upcoming participation in CMS athletics. It contains general requirements that all athletes must fulfill before they can participate in any sports program.


A. Required Forms:

Before they can practice:
1. A physical or physical update. They must be signed by a physician.
2. Proof of insurance. Either a signed waiver or School Insurance.
3. Medical emergency consent form. This form must be signed by a parent.
4. Safety guidelines form.


Before they can play in a game or match.
1. An ASB card must be purchased by athlete.
2. A Participation Fee of $20.00 paid to the Middle School office.
3. Athletic Contract: Signed by a parent and the player.

B. Eligibility Requirements:
District Policy requires that a player must have passed all of their last quarter classes and have a grade point of at least 2.00. If they do not meet this requirement, they will begin the season on academic probation and their grades will be monitored weekly.

C. Special Requirements:
Coach and Sport specific. Coaches are encouraged to have a parent informational meeting no later than the first week of practice.

D. Practice Times:
Generally, the practice schedule is from 2:20 - 4:00 or 4:30. Players are strongly encourages not to miss any of the practices.
